Helping patients leave hospital sooner.

Health and care organisations in Somerset are working together to raise awareness of the benefits of patients recovering at home as soon as they’re well enough to leave hospital.

The campaign ‘There’s No Place Like Home’ has been launched to promote the benefits of recovering at home from a hospital stay.

Health professionals say when seriously ill, hospital is the right place to be but that evidence shows that once the critical phase is over, hospital is not the best place for recovery and rehabilitation, as the surroundings are unfamiliar, mobility is limited, and good sleep is often less easy to come by.

Being ready to collect a loved one, as soon as they’re medically well enough to leave hospital can make a big difference. Even simple arrangements, such as regular visits to see a patient or, providing help with meals, can shorten a hospital stay.

And, if you’re receiving care at home for someone, who you think no longer needs the care, you’re asked to contact your care provider so their needs can be reviewed.

The number for Adult Social Care in Somerset is 0300 123 2224.
